The physician review is done against their own intake — age, allergies, and medical history — not a generic form. If something in their history needs a different medication or a substitution, that happens during review, before anything ships.
The intake is a short online form — most people manage it on their own, and you're welcome to sit with them for it if that's easier. Either way, you're not signing up for ongoing management after checkout.
No. Survival Scripts kits are cash-pay — that keeps the price of the medications and consultation as low as possible, and keeps a slow approval process out of the way.
You're not taking anything over. The physician review is between them and the reviewing doctor, based on their own intake — you're simply the one arranging it and covering the cost, the same way you might send flowers or handle a bill for someone you look out for.
Not as a surprise, and not on their behalf — the order has to be in their name, at their own address, because the physician review is based on their medical history and the pharmacy dispenses to the patient. What you can do is set it up together and pay for it. In practice that's one short conversation and about ten minutes of intake, with you covering the cost at checkout.
